Nearly 25.7 million Americans over age 40 have cataracts and the number is projected to increase to 45.6 million by 2050, according to the American Academy of Ophthalmology. Cataracts occur as a process of the body’s aging process. A cataract is caused by an accumulation of proteins in the lens of the eye. The proteins make the lens cloudy and thus cause vision loss by blocking light.
Cataract surgery is the most common elective surgery among Medicare beneficiaries in the United States. As outlined by American Academy of Ophthalmology, there are multiple studies that have demonstrated association between cataract surgery and improved quality of life, reduced risk of falling and fewer car crashes. In addition, one study found that those who had cataract surgery had a 40 percent lower long-term mortality risk than those who did not.
In traditional cataract surgery micro-incisions are created by a blade, the front of the lens is opened mechanically and the lens is broken into fragments by an ultrasonic probe. This probe then removes the dense part of the cataract and after cleaning the remaining cataract tissue an intraocular lens (IOL) is inserted through this micro-incision. No sutures are usually required because the incision is self-sealing.
The laser used in laser-assisted cataract surgery is called a femtosecond laser. It is the same type of laser used in LASIK procedure to correct near-sightedness or far-sightedness.
During laser-assisted surgery, our cataract surgeon may also use the laser to remove the front portion of the capsule that surrounds and protects the lens. They use the laser instead of ultrasound to break up the defective lens. The surgeon can also correct any astigmatism the patient has at the same time they’re performing cataract surgery.

You may not need surgery immediately, but a cataract left in place for too long becomes hard and more force is needed to break it up. This, and the manipulation of the hard fragments inside the eye, may damage nearby structures. Occasionally, the lens cannot be broken and it has to be removed as a large single piece. This requires a larger cut in the cornea which has to be closed with stitches. The post-operative course may be more difficult and the final vision may be less satisfactory than when surgery is done at the right time.
